The Promise Garden: A Symbol of Community Hope

A central moment of the morning is the poignant Promise Garden ceremony. Participants carry color-coded flower pinwheels that reflect their personal journey with dementia:

  • Blue: Someone currently living with Alzheimer's or another dementia.

  • Yellow: An active caregiver for a family member or patient.

  • Purple: Walking in loving memory of someone lost to the disease.

  • Orange: An advocate supporting the vision of a world without Alzheimer's.

Our team from Americans Home Health & Hospice Care proudly raises yellow and purple flowers. We hold yellow high for the devoted family caregivers balancing sleepless nights and daily medication routines, and purple to remember the cherished patients we have had the honor of guiding through palliative and hospice journeys.

Navigating Alzheimer’s: The Value of In-Home Health and Hospice Support

Alzheimer's is a progressive journey that impacts the entire family unit. As the condition evolves, having professional, compassionate clinical assistance at home prevents caregiver burnout and reduces emergency hospital visits.

1. In-Home Skilled Nursing & Rehabilitation

In early to middle stages, maintaining physical mobility, cognitive safety, and routine is essential. In-home physical, speech, and occupational therapies help patients retain daily function while keeping living spaces safe from fall hazards.

2. Respite & Personal Care for Caregivers

Caring for a parent or spouse with memory loss is physically demanding. Routine home health visits provide family caregivers with needed relief, education on behavioral changes, and specialized support.

3. Compassionate Hospice Care in Late-Stage Dementia

When curative treatments no longer apply and comfort is the top priority, specialized in-home hospice care surrounds the patient and family with holistic care. This includes specialized pain and symptom relief, emotional and spiritual counseling, and 24/7 on-call nursing support—ensuring dignity in familiar, loving surroundings.
